Organising and maintaining your home environment.

Help to manage medication, prompt medication, support with the re-ordering of repeat prescriptions.

Support when accessing the community or when attending appointments.

Accompanying on therapeutic activities.

Support with food shopping, food preparation, paying utility bills, helping with written correspondence.

Helping you to make important decisions.

Prompting with personal care.

Support you to ring up and report repairs to your landlord when needed.

All Liver Care and Housing staff, encourage service users to actively participate in their own support plans, and aim to promote maximum level of functioning and social inclusion.

Referrals

We accept referrals from local authorities and registered charities.

Local Authorities will undertake their own housing needs assessment prior to referring a service user to our service.

Each assessment will also have evidence of the service users specific need for accommodation.

We will then meet the service user and together, we will discuss and decide if the placement with our service is right for the individual.
